Pets Form TATCC Document - Pets Form Use this form as a supplement to your tenancy agreement if you wish to allow your tenant to keep one or two pets. Document title*Please give a title for this document (e.g. Pet form re John Smith tennacy of 1A Acacia Avenue). This title will appear as the document name in your My Documents area, but not on the document itself. If you are viewing this after clicking the 'duplicate' button in My Documents, you should give your document a new name to prevent confusion.Landlords name*Put the landlords full name here. It must be the same as on the tenancy agreement. Tenants name*Again, this must be the same as on the tenancy agreement. So if there is more than one tenant named on the tenancy agreement you need to put all the names. The property address*This needs to be exactly the same as in the tenancy agreement. If you are renting a room in a shared house, make sure you give the room details as well as the address. For example: Room 1, 23 The Street, Anytown, Anycounty AA1 2BB The date of the tenancy agreement DD dash MM dash YYYY This needs to be the date of the tenancy agreement this pet agreement is referring to. If you are signing this up before the main tenancy agreement is signed, leave this field blank and write it in later. Pet Information Please try to complete all the fields. Vets addressVets telephone NoVets out of hours telephone NoNot essential but a good idea.Looking after the pet in an emergency The next set of details is for someone who will be prepared to look after the pet in an emergency. It is strongly advised that you ONLY accept tenants with pets where this can be provided. Otherwise, you may end up having to look after the animal yourself, for example, if the tenant has an accident. Be aware that if the tenant is not available (e.g. if he has an accident and is in hospital) and if there is no carer, YOU will be responsible for the animal. This does not necessarily mean that you will have to look after it yourself. However, at the very least you will need to contact an animal welfare organisation such as the RSPCA or make arrangements with a friendly neighbour to arrange for its care. You should try to speak to the carer before this agreement is signed, to confirm that they understand that this agreement is being made and are willing to look after the animal if necessary. Make a file note of your telephone conversation and keep it with your paperwork. Carers name*Get the full name if possible. Eg Mr Andrew Smith rather than just A Smith.Carers address*This should be reasonably close to the property. Someone living in Aberdeen if the property (and the pet) will be in Brighton is not satisfactory! Carers contact tel*If possible try to get at least two telephone numbers. Carer other contact detailsUse this for an email address, skype number etc. it is important that you will be able to contact this person in an emergency.Is this Pet Form being used to amend an existing tenancy agreement?* Yes - this is an amendment No - this is being used with a new tenancy agreement If so you can make a charge and grant permission on the basis that the rent will be increased. However, this does not apply if the form is being created to use with a new tenancy agreement. Rent increaseIf you will be increasing the rent, please put the new rent in the box - for example. £1200 per month If you are not going to increase the rent, leave the default N/A.Additional ClausesN/AIf there are any additional clauses you want to add to this agreement you should add them here (starting at clause number 17, or 19 if the agreement is amending an existing tenancy). If you do not add anything here, the form will state N/A by default. Guidance Notes Ideally, this agreement should be signed at the same time as the main tenancy agreement. If this is done the tenancy agreement needs to refer to this agreement and state in the additional clauses section that the pets clause is amended as set out in the annexed Pet Agreement and Information Form. If this agreement is signed afterwards (for example if an existing tenant requests leave to keep a pet) then there is no need to amend the original tenancy agreement as this agreement serves as an amendment document. Before this form is signed make sure the date of the tenancy agreement which is being amended is included. You will need two copies of this agreement. Both parties should sign at least one copy. Then a copy signed by the tenant should be retained by the landlord (and kept carefully with the tenancy paperwork) and a copy signed by the landlord (or agent on his behalf) should be given to the tenant. If the agent signs on behalf of the landlord, you should add the words 'As agent for the landlord' next to the signature. Now check that all the information is correct and click to submit.
